this form allows parents to designate a standby guardian. the standby guardianship designation form enables the parent to plan for the future care of a child(ren), without terminating parental or legal rights, and to give the standby guardian the authority to act in a manner consistent with the known wishes of the designating parent regarding the care, custody, and support of the minor child(ren). the form must also be signed by the standby guardian and alternate standby guardian (if applicable).
