this is a limited type of principal authority and can only be used for certain matters with the dmv in nevada. due to the authority being delivered to the agent, the information entered must match what the state of nevada has on the books to identify the agent, the principal, and the principal vehicle. you will need information identifying the concerned vehicle owner, vehicle, and agent. look for the first statement, beginning with the words “that the undersigned,” then enter the principal vehicle owner’s full name on the first blank space. on the next line, enter the county and state where the principal vehicle owner lives on the blank spaces presented just after the terms “…county of” and “state of” (respectively).
